State Road 434 Winter Springs, Florida 32708 NFfNM%1 RE: State Road 4341Tuscora Drive Intersection Improvements Proposal for Professional Engineering Services — Plans Update (Supplemental Agreement #1) Dear Mr. Fields: Johnson, Mirmiran & Thompson, Inc. (JMT) is pleased to present this Supplemental Agreement #1 and the associated fee proposal for the SR 434/Tuscora Drive intersection plans update as detailed in the Scope of Services below. The terms of this proposal are consistent with our Continuing Engineering Services contract terms and conditions, including manhours and hourly rates, SCOPE OF SERVICES Plans Update and Permitting 1. Administration — Coordinate with FDOT, City and Utility owners and attend field meetings (FDOT — 1 mtg and UAOs —1 mtg) 2. Utility Coordination and Design Perform utility coordination (meetings, site visits, and data transmittal). Utility owners include Bright House Networks (CATV, Phone Lines - Fiber), City of Winter Springs (Water, Sewer, Reclaim, Stormwater), Progress Energy (Transmission & Distribution) and AT &T /Distribution (Phone). • Update all utility adjustment sheets based on input from utility owners. Based on the Roadway Construction Plans (100% Plans Submittal on January 2010) prepared by JMT, the below additional improvements will include in the plans update: • 12 -inch diameter water main extension approx. 250 -feet on SR 434 • Extend 12 -inch diameter force main through existing casing to the north and connect with existing 12 -inch diameter ductile iron pipe to the south. • Adjust for conflicts with sanitary and storm sewer at intersection of SR 434 and Tuscora Drive • Adjust for conflicts with FM at intersection of SR 434 and Tuscora Drive Designs will be in accordance with FDOT and City of Winter Springs Water and Sewer Design and Construction Specifications. Quantities for 12" and 8" water and sewer will be listed separately on the plans. City standard details will be used and shown on the plans. Frontend documents will be those originally included with the scope — no separate front end documents will be provided. Technical specifications for construction of water and sewer will be City standard. No hydraulic model will be provided. Calculations, other than those for determining restrained joints, will not be provided. No additional test holes are anticipated or included for connection points. The water and force main is to be shown on the road improvement plan, profile and cross section sheets, or separate plan /profile sheets may be included. Separate detail sheet for details will be provided. No other main size determination or analysis will be provided. 3. Roadway Plans — N/A 4, Drainage Design — N/A 5. Signing and Pavement Marking Plans — N/A 6. Signalization Plans — N/A 7. Permitting — Includes coordination with and plans review /approval by FDOT. Coordinate with FDOT for connection and utility permits 8. Design Survey — N/A 9. Geotechnical Investigations — N/A 10. Environmental Permitting Services — NIA 11. Post Design Services — Includes Pre -bid meeting, coordination with the City and CEI, and support on RFIs. Deliverable 2 set of plans (11x17) —1 signed and sealed (original) and 1 copy 1 CD includes electronic files in PDF City's Responsibility Provide and assist with FDOT Coordination for plans approval. Provide administration and coordination with local residents on the plans update. SCHEDULE A thirty (30) day project schedule is anticipated to complete the construction drawings. This time is exclusive of any review time by FDOT, and the utility owners. Close coordination will be required in order to minimize the review time by the various agencies. www.jmt.com PROFESSIONAL FEES Design, Construction Plans Preparation and Permitting ............... .....................$14,488.40 The attached sheet provides a breakdown of -the man -hour and fees. Exclusions and Assumptions The aforementioned scope of services assumes that: 1. If necessary, Soil contamination mitigation will be dealt with by others. 2. No Right of Way takes are required. 3. No schedule and Construction Estimate are required. 4. If required, a sub - surface utility exploration will be performed by the City and results provided to JMT. 5.
