The City of Winter Springs has elected to extend the abovementioned agreement for an
additional (1) year term as per Item 2.1 - Term and Definitions, which is located on Section
00500 of the original Agreement Form.
This extension shall be from May 14, 2010 to Mal 15. 2011 and shall be bound by all
covenants, conditions, and addenda of the original agreement. Base pricing shall not exceed a
total of 818"300 (E/ghleen Thousand Three Hundred and 00/100 Dollars) per year for
maintenance services as defined in the agreement Fountain Maintenance Program —
Various Areas, dated May 15, 2009, between the City of Winter Springs and Fountain Doctors,
LLC. This is the first renewal for this contract agreement.
The City of Winter Springs has been sab'sfied with maintenance services provided by
Fountain Doctors, LLC, for This agreement performed during the past year without
excelpTion.
This extension agreement is accepted according to the following:
