Correction
Dear Ms. Pilcher:
It has recently come to our attention that the amount of your survivor annuity benefit was not
correctly calculated. Your late husband Carl elected the 50% joint and survivor annuity benefit
option. Under this option, Carl's monthly benefit was $1017.39 payable during his lifetime, and
upon his death the correct amount of the survivor benefit payable to you is 50% of the monthly
benefit paid to Carl, or $508.70.
Unfortunately, the survivor benefit amount that has been paid to you (in error) since Carl's death
in July 2010 is the same monthly benefit that Carl received -- $1017.39. As noted above, the
correct amount of the survivor annuity benefit is $508.70 per month.
It is necessary that we correct this mistake. Accordingly, beginning with your next benefit
payment, the amount of your monthly survivor benefit payment will be $508.70. In addition, it
will be necessary for you to pay back the overpayments made over the past six months totaling
$3052.20. Unless we hear from you differently, we will deduct 127.17 from your monthly
benefit each month for the next 23 months, and $127.29 for the 24 month, to make up for the
overpayment.
We are sorry that this action is necessary, but I am sure you understand that errors of this type
