Revised Public Input Form
SUMMARY
Section 2-28(k), Winter Springs City Code requires teh City Clerk to create and maintain a
short public input form, subject to the City Commission's approval. The last revision was
approved in 2021. The proposed public input form removes the "Address" line following
the adoption of Ordinance 2025-14 providing that individuals addressing the Commission
state only whether they are a resident of the city. Also included in this version is a
checkbox to indicate whether the speaker is a Winter Springs resdient, if the speaker is
representing a group, and a line to identify the name of any group being represented.

Staff is requesting the City Commission review and approve the revised Public Input Form
