REQUEST: The Parks and Recreation Department wishes to inform the City Commission that
it plans to conduct a Summer Day Camp Program at the Sunshine Park Civic Center.
PURPOSE: To inform the Commission that the Parks and Recreation Department will conduct
a Summer Day Camp Program again this Summer at the Sunshine Park Civic Center.
CONSIDERATIONS:
. The Parks and Recreation Department successfully conducted a summer day camp program in
1999, 2000 and 2001.
. The Sunshine Park Civic Center is available to schedule a summer day camp program.
. Parks and Recreation will closely monitor income and expenses to ensure completion of the
program within available revenues.
FUNDING:
Funding is budgeted in the amount of $ 46,000 for the Summer Day Camp Program.
Total revenue is estimated at $ 50,400.

General Rules and Regulations
All parents will sign the Registration form, stating that they understand and
will follow the General Rules and Reoulations of the Summer Camp. The
information below fully illustrates the General Rules and Reoulations
parent(s)/Iegal guardian(s) must follow.
1. The parent(s)/Iegal guardian(s) must sign out their child each day. No one
is permitted to leave the facility or pick up a child without written parental
consent. The City of Winter Springs does not allow visiting family and
friends to attend our summer camp for the day.
2. Each child is required to wear a shirt, shorts, and athletic shoes each day.
Towels, swim suits, and water bottles are needed each day. Children
must wear a Camp T-shirt over their swimsuits when in the pool.
*** Sunscreen should be applied at home each morning - 80% of our day
is spent outdoors. ***
3. Please label belongings.
4. Children must bring a non-perishable lunch and drink each day. We have
two scheduled snack times each day. Snacks are available with items
ranging from $.50 to $1.00. Parents will be notified when lunch will be
included with the day's field trip.
5. Children must remain with their groups at all times. Any behavior that
might cause an accident or hurt others will not be tolerated. If your child is
asked to leave camp due to disciplinary reasons, there are NO refunds.
6. Do not send sick children to camp. If a child becomes ill during camp
Hours the parent(s)/Iegal guardian(s) will be required to pick up sick
children. Staff is not permitted to administer medicine of any kind. If your
child is asked to leave camp due to disciplinary reasons, there are NO
refunds.
7. The City of Winter Springs is not responsible for items lost, damaged or
stolen at Sunshine Park or during field trips. Children cannot bring games
or toys to camp.
In addition, parents have signed that they understand there is $5.00 per 5 minute
fee charged for children NOT picked up by 6:00 PM. In the same matter,
children should NOT be permitted into the facility or considered our responsibility
until 7:30 AM, These are the general rules and regulations. Good sound
judgement should always be applied.

I hereby request that City of Winter Springs Parks and Recreation Department RESERVE a space for my child for
the weekes) CIRCLED above. I AGREE TO PAY THE BALANCE DUE FOR ANY OF THE WEEKLY CAMP
FEES (on or before each Monday) for the weeks indicated. I acknowledge the deposit to reserve any additional space
is a $10.00 non-refundable charge. Also, I agree to pay A LATE PICK-UP FEE OF $5.00 FOR EACH 10-
MINUTE PERIOD after 6:00PM until my child has departed (pick-up). I understand ifmy child does not attend
camp on certain days the fees are not pro-rated and NO REFUNDS are made for disciplinary dismissals.
