2/7/03 -The crack weeds continue to be a problem although the situation is better than last
August/September. Maintenance regarding insecticide spraying has not apparently been
completed from visual observation. Will be requesting proof that this responsibility of the
contract has been implemented. Currently preparing a contractor list of improvements.
The overall maintenance condition is below par but the plant material is healthy. The
exception is the turf conditions. The turf has not been maintained properly in terms of
herbicide and insecticide schedules. The extent has is not detrimental but weeds have been
enabled to move in.
1/31/03 -Several area that have had small strips of sod that have done poorly have been
added to the landscape improvement list (i.e. -end of median at Oak Forest). Ornamental
grasses looking ok through the Winter months. Not a lot of maintenance action this month.
1/24/03 -Turf is dormant and non-turf plants locations are obvious. They are not wide spread
but are evident on every median. The ant problem has not been addressed at all.

1. Add to clean up cost remove ant pile on Tuskawilla road
01/03/03: Ant beds remain. No report on turf. No chemical report yet. Will schedule an
appointment in the field to discuss.
12/27/02 - Dormant growth season. Turf decline issue still a problem in the medians.

12/12/02 -
1. Ant beds remain. New ones have cropped up.
2. Turf continues to decline.
3. DEADLINE FOR ANNUAL REPLACEMENT HAS BEEN MISSED. Repeated verbal
directives have not been met to place the Christmas season annuals. Grady: In by
tomorrow.
4. Fertilizer report from SEPTEMBER has still not been turned in as requested
repeatedly.
12/05/02 -Severe turf invasion problems at medians. Ants still not under control. Trimming
needs to occur on the Crape Myrtles.
11/27/02 -Ant beds not improving. ROW areas.
11/21/02 -Discussed Indian Hawthorne needs to be trimmed at Oak F. planters, Winter
annual schedule: In by Dec. 12th; NEED FERTILIZATION REPORT!!!.

Please Note: Specified areas in the contract -Mulching, Herbicide and Fertilizer schedule

1. Tend to ant mount at the base of holly trees between Trotwood blvd and winter springs
blvd
2. Some medians have dollar weeds on the east side of the two medians before and after
winter springs blvd there are brown patches in the turf. Report back on dollar weed
remedy immediately
3. Medians at Trotwood blvd entrance grass in brown and dead please investigate
