September 20, 2004 To: All Hurricane Charley and Frances Clients Re: Amended Proposal for Hazardous Stump Removal (Revised) In our continuing effort to better support all applicant's in their recovery from the above mentioned storms and our further commitment to the affected communities, Grubbs Emergency Services, LLC respectfully submits the following revision to our proposed ~tump pricing. We have recently received what appears to be guidance, not policy, regarding acceptable price ranges for hazardous stump removal and in order to comply with the current disaster environment, the prices listed below shall apply for this operational element. The removal of hazardous stumps are a unique process requiring specialized equipment. As such, this process requires unique documentation and costing. The CLIENT and CONTRACTOR will measure each stump three (3) feet above normal ground level, to determine the diameter ofthe trunk. Once the diameter is established, the stump will be physically removed by the best means available. The stump will be photo documented by the CLIENT and recorded on a specific log provided by the CONTRACTOR. (Anything below 12" in diameter will be treated as curbside debris.) The CONTRACTOR shall invoice the CLIENT for the removal of stumps based on the categories below: 12 inch diameter and up, but less than 24 inches- $ 650.00 per stump 24 inch diameter and up, but less than 48 inches- $ 850.00 per stump Equal to or greater than 48 inches $1,200.00 per stump This price includes the pick up, hauling and reduction of the stumps. Ifnecessary and requested, the contractor shall invoice the CLIENT Ten Dollars and 00/100 ($10.00) per truck measured cubic yard for acquiring, hauling, placing back-fill material to level holes that pose an . ....- immediate threats to public to life, health and safety of the community. The fill will be quantified with tickets provided by our company. Please be advised that these reduced rates are a reflection of the current disaster guidance regarding stumps and should not be con~idered to be policy for all disasters. Contract Addendum SCOPE-OF -SERVICES City of Winter Springs Proiect Descriotion Hurricane Charley/Ftances deposited debris, mostly vegetative, into public parks . and recreation areas, public rights of way, drainage easements, small creeks, and as well as other public property to include undeveloped low lands near, or adjacent to urban development. Some of these natural and man-made areas proceed through private property. Failure to address the significant blow down of debris will threaten improved property and will remain a public health and safety threat to the community and to future flooding and, during periods of drought, fires resulting from the build up of ground fuels. This addendum describes the work methods and responsibilities to complete the work. Scooe . Contractor will provide all necessary labor, equipment; and materials to effect a safe and efficient removal and disposal operation. Means and methods are .at the discretion of the contractor, but all work should be to abate immediate life, health and safety hazards designed to minimize impacts to the community, to include but not limited to, wetlands and environmentally sensitive areas. Actual work will be at the direction of the City of Winter Springs. In rural or remote areas, Contractor will cut identified trees. The debris generated will be placed in the right of way and removed under the current cubic yardage contract. Where practical from creeks; streams, waterways, etc... materials will be placed on stream banks (or at locations outside the channel flow). Such material shall be secured in a manner to minimize the likelihood that the material will float back into the streambed during periods of high water. Where this method is not practicable, materials will be removed, hauled, and reduced under the current cubic yardage contract. Within utban areas, materials designated by the City of Winter Springs 'will be removed, hauled to a staging area, and reduced under the current cubic yardage - . contract. ; ,-......1. ........ Responsibilities City of Winter Springs: . . Identify "geographic" areas for all work by providing Contractor with a digitized map oflocations and mcluding an overlay of identified floodplains. (Contractor will maintain and update the map upon receipt). l) Identify easements and ingress/egress routes for equipment, areas with limited access may require additional costing based on logistical challenges present. o Provide a monitor for each crew. o Identify, either by marking or direction of the monitor, those materials to be removed. tl Maintain "unit price" logs and write load tickets. tl Rate loads hauled to a staging area. t! Provide necessary permits prior to work in waterway's and wetlands. t! Work areas will be identified at the City of Winter Springs's discretion to ensure material is placed in an area that will not create further hazards. .. Provide access to all work areas (public or private). City of Winter Springs must execute all necessary forms (right of entry) to allow contractor access onto private property. Once the proper forms are executed, the City of Winter Springs shall provide the Contractor with a copy prior to work commencing. Contractor . Provide quality control inspectors. . Develop a schedule and direct crews. . Maintiifu a map designating areas of completion and provide City of Winter Springs with regular reports on progress. . J . Documentation and Fees Work under this addendum will be on a unit price basis. Costs for work within identified areas are listed on attachment "B". Hauling and reduction is conducted under the current cubic yardage contract. ( , . . . ' , A IT ACHMENT "B" Tree Removal from Rural or Remote Areas The removal of both down and leaning trees that threaten public areas and rights of way or the natural flow of water and threatens flooding of improved property is a unique 'process requiring specialized equipment. As such, this process requires unique documentation and costing. The CITY OF WINTER SPRINGS and CONTRACTOR will measure each tree three (3) feet above normal ground level, to determine the diameter of the trunk. Once the diameter is established, the tree will be physically removed by the best means available. The tree will be photo documented by the CITY OF WINTER SPRINGS and recorded on a specific log provided by the CONTRACTOR. Once the tree is removed or cut into manageable portions; the tree will be placed in a manner so to not accumulate on the rights of way, creek bal1ks, etc.. .so not to create safety hazard or accumulate during high water situations. The CONTRACTOR shall invoice the CITY OF WINTER SPRINGS for the removal of dangerous or leaning trees and placement in a safe area, for removal under the current cubic yardage contract: - From" j lnches and up, but less than 6 inch diameter- $ 45.00 per tree . - -..- 6 inch diameter and up, but less than 12 inches- $ 85.00 per tree 12 inch diameter and up, but less than 24 inches- $ 165.00 per tree 24 inch diameter and up~ but less than 48 inches- $ 470.00 per tree Equal to or greater than 48" $ 700.00 per tree The removal of hanging or broken limbs (Greater than 2;' in diameter and 2 feet or greater in length) will be conducted for a cost of$ 150.00 per tree (no matter how many limbs are removed) up to a height of 65 feet, at the City of Winter Springs's discretion. Tree Removal in Urban Areas The removal of both doWn and leaning trees that threaten public areas and rights of way or the natural flow of water and threatens flooding of improved property is a unique process requiring specialized equipment. As such, this process requires unique documentation and costing. The CITY OF WINTER SPRINGS and CONTRACTOR will measure each tree three (3) feet above normal ground level, to determine the diameter of the trunk. Once the diameter is established, the tree . will be physically removed by the best.means available. The tree will be photo documented by the CITY OF WINTER SPRINGS and recorded on a specific log provided by the CONTRACTOR. Once the tree is removed or cut into manageable portions, the tree will be removed to a staging area on site to be loaded and hauled toa TDSRS for processing. The loading, hauling and processing of the trees removed to the staging area will be conducted under our cUrrent curbside collection operations. The CONTRACTOR shall invoice the CITY OF WINTER SPRINGS for the removal of dangerous or leaning trees and placement at an on site staging area for removal under the curbside collection portion of our contract: . , From 3 inches and up, but less than 6 inch in diameter $55.00 per tree 6 inch diameter and up, but less than 12 inches- $90.00 per tree 12 inch diameter and up, but not less than 12 inches- $190.00 per tree 24 inch diameter and up, but less than 48 inches- $545.00 per tree . Equal to or greater than 48 inches- $850.00 per tree The removal of hanging or broken limbs (Greater than, 2" in diameter and 2 feetor greater in length) will be conducted for a cost of$150.00 per tree (no matter how many limbs are removed) up to a height of 75 feet, at the' City of Winter Springs discretion. The limbs will be removed to an on site staging area for removal Wlder the curbside collection portion of our contract. Stump Removal from Urban Areas The removal of hazardous stumps is a unique process requiring specialized equipment. As such, this process requires Uhique documentation and costing. THE CITY OF WINTER SPRINGS and CONTRACTOR will measure each stump three (3) feet above normal ground level, to determine the diameter of the trunk. Once the diameter is established, the stump will be physically removed by the best means available. The stump will be photo documented by the City of Winter Springs and recorded on a specific log provided by the CONTRACTOR. Once the stump is removed or cut into manageable portions, the stump will be loaded and hauled to a staging area for reduction through grinding. The CONTRACTOR shall invoice the CITY OF WINTER SPRINGS for the removal of hazardous stumps as wen as the loading, hauling and reduction. From 3 inches and up, but less than 6 inch in diameter $175.00 per stump 6 inch diameter and up, but less than 12 inches- $350.00 per stump 12 inch diameter and up, but not less than 12 inches- $700.00 per stump 24 irtch diameter and up, but less than 48 inches- $1,050.00 per stump Equal to or greater than 48 inches-_ $1,400.00 per stump The CITY OF WINTER SPRINGS reserves the right to terminate this agreement at any time and at their discretion.
