Dear Mr. Pula:
Thank you for calling Environmental Services, Inc. (ESI). Weare pleased to provide you with a
proposal to conduCt a :Phase I Enyironmental Site Assessment (E,SA.) .on the above-r.efe.renced
property in ":Seminole. County, Florida;;.. The -p.hase; LEnvironmental SIte Assessment will be
conducted in conformance with the scope and limitations of the Americ~n SOclety for' 'testing
and Materials.(ASTM) Standard Practice E1527-00;
The Phase I Environmental Site Assessment will include the following:
0 Inspection of site and adjoining/nearby vicinity, including inspection of the site for
evidence of recognized environmental conditions, as defined by ASTM Standard
Practice E1527-00, which includes only the identification of petroleum product and
hazardous substance contamination concerns and does not include other potential
concerns such as radon and asbestos unless those tasks are authorized as additional
assessment task(s);
0 Review of past occupant/owner activity and history based on interviews with present
and past owners and/or occupants of the property, a review of any available historical
city directories and fire insurance maps;
0 Review of historical aerial photographs on file in the Seminole County Engineering
Office; .

0 Survey of reasonably asccrtainablepublic records (ASTM~designated federal and
state databases, as well as additional research of county regulatory agency files for
storage tanks, hazardous waste generators and dump sites) for listedracilities,

including registered storage tank facilities, hazardous waste generator facilities, and
contaminated sites; and
0 Summary of existing conditions in a written report with site photographs and other
pertinent documentation indicating presence (or possible presence) of recognized
environmental conditions as defined in ASTM Practice EI527-00.
The above Phase I tasks will be completed for a fee of $1,800.00 for the study, barring the
encountering of unforeseen site circumstances, If such unforeseen circumstances occur, you will
be notified immediately and no work will be performed that exceeds the above cost estimate
without your prior authorization. Additional meetings, consultation, other services requested or
required by the client or others (e.g., client representatives, attorneys, lenders) beyond the
outlined scope of services will be invoiced on a time and materials basis and may cause the
above cost estimate to be exceeded, If you require specific guidelines for conducting the
Phase I ESA, please advise ESI; otherwise, the above listed guidelines will be utilized.
In order to facilitate the completion of the Phase I ESA, ESI will need information on any
previous property uses of which you are aware; contact names and phone numbers, if known, for
present or pervious owners/occupants of the property; any knowledge of past or present use of
hazardous substances or petroleum products on the property; copies of any subsurface
investigations previously conducted on the property; and copies of any other pertinent reports or
appraisals on the property. This information can be provided to ESI through your completion of
a questionnaire that we will forward to you upon receipt of a signed proposal.
We look forward to working with you and are prepared to proceed on receipt of your
authorization, This letter will serVe as a contract between Environmental Services, Inc., and the
City of Winter Springs. Our invoices are prepared monthly for the prevIous 30 days, Payment
for services is due upon receipt and a 1.5-percent per month late fee will be assessed on unpaid
balances due over 30 days, As the client, you agree to pay any and all attorney fees that are
required to collect unpaid balances due over 60 days, All suits at law or in equity for any breach
of the agreement or for default in payment shall be instituted and maintained in any court of
competent jurisdiction in Brevard County, Florida.
