Agreement. The GENERAL CONDITIONS, SUPPLEMENTARY CONDITIONS, and
STANDARD SPECIFICATIONS apply to and govern all work under this Change Order.
Reason for Change: One tree was marked for removal in the plan set as advertised. After the
surveying and marking of the roadway and sidewalk limits, additional trees are required to be
removed for the installation of the sidewalk

WAIVER This Change Order constitutes full and mutual accord and satisfaction for the
adjustment of the Contract Price and Contract Time as a result of increases or decreases in
cost and time of performance caused directly and indirectly from the change. Acceptance of
this Waiver constitutes an agreement between OWNER and CONTRACTOR that the Change
Order represents an equitable adjustment to the Agreement and that CONTRACTOR shall
waive all rights to file a Contract Claim or claim of any nature on this Change Order. Execution
of this Change Order shall constitute CONTRACTOR's complete acceptance and satisfaction
that it is entitled to no more costs or time (direct, indirect, impact, etc.) pursuant to this Change
Order.
APPROVAL AND CHANGE ORDER AUTHORIZATION
ACKNOWLEDGMENTS
The aforementioned change, and work affected thereby, is subject to all provisions of the
original Agreement and specifically changed by this Change Order; and it is expressly
understood and agreed that the approval of the Change Order shall have no effect on the
original Agreement other than matters expressly provided herein.

Dear Greg,
Southland has prepared a cost proposal for the additional six trees which will need to be removed in order to
complete the construction of Phase 1 as designed. Please review the following sheets for a breakdown of costs
associafed to perform this work.
Southland would also like to request a four day time extension to perform this additional work. The reason,
Progress Energy will have to first mobilize a crew to remove all vegetation below the existing power lines before
our sub and crews may complete the removal work.

Tree work includes the hauling of debris generated from the removal or trimming of trees unless
specificially priced for -Cut & Leave only-. All work supervised by supervisory personnel and according
to ANSI 300A standards. Every tree has a potential of catastrophic failure; proper preventative and
maintenance pruning can help to reduce property damage. Due diligence will be taken to inform client
of potential structural failure of tree{s) on property, however. A Budget Tree Service, Inc. cannot be held
responsible for such failure if removal recommendations, proper preventative maintenance pruning is
not followed or for hidden faults. Trees that are mature and structurally unsound will suffer branch
failure or worse; we recommend that these trees be identified and monitored. Mature co-dominant
stemmed trees will split and fall - we recommend identifying them and planning the pruning so that in
the event of limb/stem/co-dominant stem failure, no structures are damaged - although we cannot
guarantee the performance in a storm/hurricane, we can most assuredly prune to minimize damage;
also -light or selective thinning- or conservative pruning is paramount to a tree's ability to produce and
store sugars for proper development, health, growth 8. performance - overpruning trees can cause poor
performance in a storm/hurricane situation, jeopardize their health and shorten their life span - research
proves trees are engineered genetically for performance in stormslhurricanes - mispruning them can
[nvite further damage and catastrophic consequences.
The above prices, specifICations and conditions are satisfactory and are hereby accepted. A Budget
Tree Service, Inc. is authorized to do the work as described above. Payment is due upon completion.
CUSTOMERS ARE RESPONSIBLE FOR ACQUIRING PERMITS & TREE REPLACEMENTS IF
REQUIRED BY LAW.
Not responsible for underground pipes, wires or irrigation. Please identify, move, relocate or mark
possible hidden, sensitive irrigation or electrical hazards/targets. Not responsible for hidden septic
