HomeMy WebLinkAbout2022 03 14 Consent 302 - Chau Medical Site Acceptance • CONSENT AGENDA ITEM 302 n, CITY COMMISSION AGENDA I MARCH 14, 2022 REGULAR MEETING 1959 TITLE Chau Medical Site Acceptance SUMMARY The approved (November 16, 2020) project, Chau Medical is Development Area 1, The Chau Medical Center includes a 10,190 SF medical office on ±1 .22 acres. This part of the development phase includes; mass grading of the entire site, master stormwater and utilities infrastructure, perimeter landscaping, and a wall located on south side of the property. The completed project includes connections on the south side of SR 434. This agenda item is for the conditional acceptance of the site improvements. The site work has been constructed in substantial compliance with the Final Engineering Plans approved by the City Commission, except as noted on the attached punch-list, Exhibit A. Complete all site work and pass the final engineering inspection. Contact the Community Development Department at customerservice(a,wintersprin sig subject line "Site Acceptance", your project name, to schedule a preliminary and/or final engineering inspection. The final inspection will occur after receipt of the record drawings from the engineer-of-record. • Construction Site Walk-through completed on February 8, 2022. • Landscape Site Walk-through completed on February 25, 2022. 2. Submit a Bill of Sale for all infrastructure being turned over to the City for ownership and maintenance (streets, utilities, sidewalks, etc.). Use the City's standard Bill of Sale form. The Bill of Sale shall be recorded by the City Clerk. Submit final documents to the Finance Department. This includes the cost sheet and the Bill of Sale. 3. Submit 2-Year Maintenance Bond, Letter of Credit, or Cash Escrow Deposit (10% of the value of the improvements to be owned and maintained by the City, as listed on the Bill of Sale) to the Finance Department. 4. Obtain Approvals from all City Departments and Seminole County Fire Department. 5. Submit FDOT Permit Certification and provide City (CCD/Engineering) copy of FDOT approval. 6. Submit a copy of the FDEP Wastewater Permit Approvals to the City (CDD/Engineering). Notes: • City Commission site acceptance (in the form of a consent agenda item at a regularly scheduled City Commission meetin1j) is required before a certificate of occupancy can be issued for any buildings • Agenda items for City Commission meetings must be prepared approximately three weeks prior to the meeting date. • If there are any questions, please email customerservice(a�wintersprin s florg, subject line "Site Acceptance", and you project name. • Additional items to be completed prior to closeout (this is not a complete punch list): 1.
