RIGHT-OF-WAY. SPECIAL PROVj$IONS 1. All construction and/or maintenance on the FDOT Right-of-Way shall conform to the Federal Manual on Uniform Traffic Control Devices for Streets and Highways, the FDOT Roadway and Traf fie Design Standards, and the Standard Specifications for Road and Bridge Construction. 2. The Permittee is responsible for insuring that each person supervising the selection, placement and maintenance of Traffic Control Devices in Utility Work Zones shall be certified by attending a FDOT approved MOT training course. A copy of these certifications shall be submitted to FDOT upon request. 3 . Site condition changes that warrant a change to the proposed maintenance of Traffic plan will require the permittee to notify the FDOT. 4. Upon notification by the FDOT of deficiencies in the Traffic Control plan or other matters involving traffic safety, the permittee shall immediately make improvements as directed by the FDOT. Should the FDOT deem conditions to be such that imminent danger is present, all work shall cease immediately until the conditions are corrected. 5. The Permittee shall notify the FDOT Construction office 48 hours in advance of starting proposed work at (407) 623-1242 Attn. Don Carmer. 6. At the time of notification the Permittee or his representative shall submit a list of emergency phone numbers and contacts in the case of any emergency. 7. The Permittee is cautioned that utilities may be located within the construction area. 8. All portions of FDOT Right-of-Way disturbed during construction shall be sodded. 9. Any landscaping, .irrigation, sidewalks, drainage connection, and utility work must be on a separate permit. 10. It is the Permittee's responsibility to meet all requirements of the Florida D.E.R. and St. Johns River Water Management District and/or South Florida Water Management District, and obtain any permits required from other governmental agencies. 11. Shoring will be required to conform with the "Florida Safe Trench Act" requirement where necessary to protect existing pavement, structures, and foundations. At such locations where FDOT signs and reflectors will interfere with proposed construction, the Permittee will notify the local maintenance office as to the type and quantity to be moved and shall carefully relocate or move them as needed. Any signs or reflectors damaged, destroyed, removed or relocated and not replaced shall be replaced by the FDOT at the expense of the Permittee. 13. A copy of the approved permit and special provisions shall be on site during all construction in FDOT Right-of-Way. Failure to have a copy of the permit on site at the time of inspection will result in a notice to stop work until such time as the permit or copy has been obtained. 14. Applicant declares that prior to the filing of this application he has ascertained the location of all existing utilities, both aerial and underground, and states that due notice of the work under application was furnished each other utility involved. ' 15. When applicable, the Permittee shall furnish compaction and density test reports by a certified lab in accordance with FDOT requirements. 16. No parking is allowed on DOT Right-of-Way while under construction. 17. According to Florida Statute 335.15, you are required to notify local law enforcement when one or more traveling lanes will be closed more than two (2) hours. 18.
