I failed to mention that the City should be cautious in requesting proof from some applicants and not from others in a way
that could infer discrimination based on some protected characteristic (race, national origin, religion, etc) ... I recommend
accepting the sworn application by itself unless you have some reason to believe there is a problem or if the applicant
voluntarily provides the information.

Joan,
As a former prosecutor, Mr. Cannon qualifies for the exemption of his personal identifying information under section
119.071(4)(d), Florida Statutes. Proceed with having him complete the form, which serves as a sworn statement as to his
qualification for the exemption. The form mentions that applicants may be required to present proof of qualification for the
exemption; however, whether to require it from applicants will need to be determined on a case -by -case basis by your
office. (I briefly searched his public profile on the Florida Bar website and my legal research program, but wasn't able to
confirm one way or another whether he was ever a prosecutor.)

A former Prosecutor named Kevin Cannon came into City Hall yesterday to request Confidentiality as he is now a
resident of the City of Winter Springs. He presently has a Law Firm in downtown Orlando at 390 N. Orange Ave.
Can we proceed with giving him our Confidentiality Form to complete so that he is Exempt? He said he is already
Exempt with the Property Appraiser's office. What identification does he need to show me?
