The following is the information that I have: Alisa Kaufman (she just got married and changed her name to Kamon last year) previously resided at 75o Tanglewood Road, Winter Springs, 32708. This address is in City limits (see attached property appraisers report for this address). She recently got married and to my guess, is renting out her house at 75o Tanglewood Road because the Property Appraisers report still says the house is owned by her. I informed her that because of her name change, she would need to re -do a W-9 for the Finance Department and she did so. I turned the W-9 original into Finance months ago. Ms. Kamon then asked that I mail everything to her post office box instead of her physical address because the post office box was where she received all her mail. I made the changes and have been mailing the agenda packets to that address ever since. The other day, Finance was reviewing the Accounts Payable and noticed that the current address that Alisa Kamon resides at was 777 Kissimmee Place, Winter Springs, 32708 by reviewing her most recent W-9. Finance also looked at the utility records and found that Alisa Kamon transferred her service from 75o Tanglewood Road to a lady by the name of Lorizette Contreras on 05/04/07. Finance looked on the Property Appraisers website and found that 777 Kissimmee Place was outside of the incorporated City limits as indicted by the Tax District on the Property Appraisers Report (see attached property appraisers report for this address). As you are aware, the Winter Springs Code of Ordinances indicates the following: "Resident" shall mean any person living within the city limits at all times while serving on said board or committee, and at least six (6) months prior to being nominated, elected or appointed to the board or committee. (c) Requirements of board and committee members. Any person nominated, elected or appointed to serve on a board or committee of the city shall satisfy the following requirements, except as otherwise provided by state or federal law: (1) Complete aboard or committee application as prescribed by the city commission; (2) Consent to a standard criminal background check; 2/5/2008 Page 2 of 2 (3) Be duly registered to vote in Seminole County; (4) Be a resident as defined in this section; and (5) Has never been convicted or found guilty, regardless of adjudication, of a felony in any jurisdiction, any plea of nolo contendere shall be considered a conviction for purposes of this paragraph. As you can see, we have a situation here that needs to be resolved. Could you please inform us as to the best way to proceed?
