The Notice-to-Proceed is effective February 26, 2015. Prior to awarding the construction contract, final concurrence from LAP Construction Project Manager, Vince Vacchiano, will be required. Additionally, this letter will serve as the Department's Notice-to-Proceed to the City with the execution of the construction engineering inspection (CEI) of the above referenced project to the numbered one firm "DRMP". The Notice-to-Proceed is effective February 26, 2015. The negotiated amount with DRMP for this project is: $160,000.70, a supplemental agreement to reduce CEI will be processed after the construction has been awarded. One fully executed LAP Agreement is enclosed, and funds in the amount of$1,391,490.00 have been encumbered for this project If you have any questions, please call E. ARS44 The Florida Department of Transportation and the City of Winter Springs desires to supplement the original Agreement entered into and executed on Februa 26t",2015 as identified above. All provisions in the original Agreement and supplements, if any, remain in effect except as expressly modified by this supplement. The changes to the Agreement and supplements, if any, are described as follows: PROJECT DESCRIPTION Name Intersection Improvements Length 2300 ft. Termini at SR434 and Tuskawilla Road Description of Work: The Project involves construction of various safety and operational improvements of the intersection of S.R. 434 and Tuskawilla Road in Winter Springs. The project also improves pedestrian safety to meet ADA and creating space for turning movements between S.R. 434 and the frontage road at the intersections. The construction of the project will include Maintenance of Traffic, milling and resurfacing, removal of Pedestrian Island and some additional demolition work, crosswalks, safety bollards, drainage improvements, concrete curb and gutter, signalization adjustments and signing & pavement markings. Construction also includes Landscaping work along with hardscape paving work including relocation of existing benches and street lights and ADA compliant sidewalks with detectable warning surfaces & ADA pavers and handicap ramps. Some coordination with adjacent FDOT project 430675- 1 -52 -01 will be required. LAP Non - reimbursable work — Project also includes relocation of Electrical Panels and transformer, seat -wall at the fountain, relocation of fountain pump and existing Publix clock, Banner poles and Entry Columns. The City will pay for these through Local funds. Termini — S.R. 434 from Doran Drive to East of Tuskawilla Road intersection length — about 2300 ft. Reason for Supplement and supporting engineering and/or cost analysis: Increase construction funding by $240,227.00, based on award to the lowest bidder in the amount of $1,538,879.00 contains $1,431,717.00 of Federal Aid eligible costs and $107,162.00 of Federal Aid ineligible costs to be paid by the City. CEI cost will be reduced by ($9,939.00) based on award amount of $190,061.00.
