whose address is Seminale County Se�-vices Building, I101 East First Street, Sanfard, Florida
32771, hereinafter referred ta as "COUNTY", and the CITY +DF WINTER SPRINGS, a Florida
Municipal Corparation,whase address is 1126 East State Road 434,Winter Springs, Florida 32708,
hereinafter referred to as"DAI�CAM�'PROVIDER".
WITNESSETH:
WHEREAS, COUNTY has received Connmunity Services Block Grant funding in the
amount af ONE HUNDRED FIFTY-N1NE THOUSAND ONE I�UNDRED THIRTY-SIX NO/lOp
DOLLAT�'.S ($159,136.00), hereina$er referred to as the Grant, from the State of Florida
Department of Economic Clpportunity for day camp schalarships for eligible indivzduals residing in
Seminole County,Florida; and
WHEREAS, in response to the needs of these individuals, COUNTY may issue vouchers
for day camp scholarships,hereinafter refened to as"Day Camp Schalarship Vouchers", for income
eligible individuals residin� in Seminale Gounty, Florida, hereinafter referred to as "Camper",
participating in a COUNTY program for payrnent af day camp tuition and ather specified expenses
for shart-terrn day camp services provided by DAY CAMP PROVIDER; and
WHEREAS,the purpase af this Agreernent is ta identify the COtINTY approved day camp
programs ta be delivered by the DAY CAMP PRt:}VIDER; establisla the costs associated with the
appraved day carnp prag-ams; describe the processes and pracedures that wili be fo]lowed if an
eligible Camper receives a Day Camp Schalarship Voucher from CUUNTY for enrallrrtent in one

CAMP PROVIDER agree as follows:
Sectic�x� 1. Term. Thia Agreement becames effective upon execution by all pariies and

Agre�ement.
Section 2. Responsibilities. CQUNT'Y and DAY CAMP PRU'VIDER hereby specifically
agree ta the fo]lowing:
{a) COtTNTY sh�ll solely deteimine v�e#her Seminole �County resider�ts iutereste.ti in
day camp servic�s are eligible for Day Camp Schalarslup Vouchers iza accardance witb applica�le
Federal and State incc�rne e7igibi�ity and other program c�riteria, including the Scope af Sexvices
attacherl hereta and incorporated h�rein as Exhibit A, Only those Campers determined eligible and
in need of day camp servic�s by GOUP�ITY shall be issueci a Day Camp Scholarship Voucher �or
day eamp services; provided,haw�ver, sufficient funds are available,�n COT.TNTY's determination,
to cover CClUNTY's portion of the day c�amp costs. All tecros and c�nditions of this Agreement are
subject to contiaued Grant iiund availability.
(b) COLTNTY shall not guarantee any referral�, set aside any day camp scholarships, or
budget any funds whatsoever for any day camp programs off�erecl by the DAY CAMP PRC}VIDER
All decisions regarding the issuance of a Day Camp Scholarship Voucher shall be znade on a case
by case basis by COLTNTY, taking into cansideration the inforrna�tian available, including, but nat
lizr�ited to the assess� neec�s af the Cam� and any additional costs to the Camper. COLTNTY

Camper. '
(�� DAY GAMP PROVIDER agrees to accegt the Day Camp Scholarship Voucher�and
bill COCTNTY for the agreed upan day camp oosts at no more than the tatal of the Day Camp
Schalarship Voucher or the total pubiished cost of the specific day catt�� setvi�es for municipai
residents,whichever is less. The arnount of tl�e Day Camp Scholarship Voucher shall be determined
by COLTI�TTY up to a u�znum amaunt of EIGHT HUI�TDRED TEN AI�ID N0/100 DOLLARS
($81(}.OQ)c►f the cost c►f day camp tuition per Cam�er to DAY CAMP PROVIDER. C011NTY shall
pay no other casts or fees that are anot specifieci in this Agreem+�a:t and included on the]Jay Camp
Scholarship Vaucher. Payments for tuition andJor other appraveci cflsts wi11 be made only for thase
Campers r+�ceiving a valid signed GOUIVTY Day�Camp Scl�olarahip�auch�r. Th�Gamper must be
enrollecl in and receiving day c�tnp se�rvices designated on the Day Camp Schalarship Voucher
before the C{)UNTY Day Camp Scholarshig Voucher is cansiderecl valid. If'the Camper does not
begin day camp, DAY CAMP PROjIIDER shall immediat�ly return tiie Day Camp SchoIarship
Voucher ta C�tINTY. COUAT'l'�will not pay DAY CAMP PRf3VIDER ft�r a week in which the
Camper dc�as not attend at least one(i)day.

CA11�P PR4VIDER allow a COUNTY Camp�r to switc� day ca�np progr�uns or to extertd hzsJher
day eamp services at CQUNTY's cost.
(h) I�AY CAMP PRt]VIDER shal� provide COLJN'FY a record of comp�etion of the
day camp program by ths Ca�nnnper within ten (1Q� business days of said completion. This '
infarmaticrn shall be released to CQUNTY based on Camper's authorization as outlined or�the Day
Camp Sclaolarsship Vaucher. DAY CAMP PRO�JIDER, shall provide COUNTY a completed
Monihly Report within ten (10} days of the end'af the month. A eapy of the lvionthly Report is ,
attached hereto and incorporated herein as E�tibit C.
(i) DAY GAMP 1'ROVIDER shall ref�r CC�Ll1tiTTY Catnpers back to CCIUNTY in the
e�vent that the Camper or DAY CAMP PR(�VTDER is unable to cantinue the da�+ camp services
specified in the Day Camp Schalarship Vaucher. DAY CAMP P�OVIDER shall alsa notify
COtTNTY in writing af this occurr�ce.
(j} L7�AY CAMP PRfJVIDER agrees that each Gamp�r receiving a Day Camp
Schcrlarshi�Voucher fo�r day camp services sha11 be evaluated by the C►AY CA.MP PRQVIDER far '
all financial aid eligibility narniaity available to DAY CAMP PR.OVITaER's Casnp�ers. A Camper
eligibte for assistance shail not be discriminated against in receipt of su.ch financial aid salely due to
hisllier rec�ipt of a COi.TNTY Day Carnp Scholarship Voucher.

S�etion 3. Payment�
(a� DAY CAMP�'RCIVIDER shall sub�it to Ct�Uhl'1"�a written izivoice in�the fc�rm of
Exhibit E, Request for P'ayment, and all neces�ary supporkit�g do�nentatic�n for fihe appr4vet�i dxy
camp tui�ion and oth�r identified and included costs �nce a Camper possessing a valid sign�d �ay
Camp Scholarship Voucher is enrc►lleti in a day camp grogra�n. This inv+�ice must be suprpc�rtec�by a
copy of the C(�UNTY� Day Camg Scholarshig Voucher ar�d any xeeeipts, as applicable. Each
CAMPER shall be identified by narne and costs associated with each Camper shall be itemized.The
payment reguest shall be addressecl to:

(b) The COtTNT'Y Day Camp Scholazship Voucher shal] specify the e7cpenses that will
be p�id fc►r witii funds admini�tered by COtJNTY. DAY �AMMi�" P�OVIDER sha1l adhere ta the
plan of action establishedby the Day Camp Schalarsh�p Voucher.
Section 4. Refunds.
(a) The DAY CAMP PROVIT7ER sha11, in confoatmity with its establish�d refvnd '
policies, refiznd such monies far day camp tuition and fees paid by COUNTY for Campers who
enroll in and begin a day camp program,but dr+ap out of day camp. Refunds due for tuition and fees

(b) In accordance with Section 2I534(2), Florids Statutes, if a eheck c�r 4ther draft is
rehirned to CC►UNT�i�''for collection,DA:Y CAMP PR�VIDER sha11 pay to CQLJNTY a service fee
Qf FIF'1'EEN AND I�C?/lOQ DOLLARS (�15.0(}) or five percent (5%} of the face amount af the
reh�.med check ar draf�,whichever is gre�ter.
�e�cction 5. Access attd Retentio�of P`ublic Recards.
(a) The parties shall cornply�ith the public rec�rds l�aws of the�tate af Florida and the
nan-confidential portions af all financial recarc�s,suppozti�ng dacuments, and any t�ther documents
pertinent to thi� Agreement shall be available for inspection by any person in accordance vuith
Chapter 119,Ftorida Statutes. DAY CAMP PI�.OVIDER sh:all comply with Fexieral and State law to
et�sure the aanf r�entiality of pers4nal infarmation including name, Sacial Security Number, Social
Security benefit data, date of birth, official Stat� ar government issued driver's license or
identificai�iion n�unber, aiien registratian nurnber, govemment passport number, emplayer or
ta�cp�tyer identifica�ion num'�er, hame address, phone nurnber and medicai infarrnation of each
Catnper receivin,g a COtTNTY Day Camp Scholarship Voucher.
{b) DAY CAMP PRQ�TIDER �hall have in place an official pz�c:►cess far investigating
and reporting lost or stolen confid�ztiai informatio�a. Any infraction or pote�tia� infraction of State
and/or Federal laws related ta the lass ar theft of confidential iz�formation shall immediately be
reported to the C4UNTY in writing. '

{c) The parties heretc� sha1l retaira ail financial r�cords, suppo�ing documents, and any
other doct�men�,s pertin�nt to this Agreeznent fc�r a period of five{5} years after the tennination or
ex��iration date, whichever is e.arlier, or such long�r period as m�y be requirc�,i by Federal t�r State
law.
Section 6. 1'�otices. Al� nt�tices, consen�s, appmvals and required reports which any
party shall be xequiretl c�r sh�ll desire to malce or give�aursuant t4 this Agreertient, shall!�e given

Section?, Termination.
{a} Th�s Agreement may be termivaLed for convenience at any time by either party with
thir#y {30) days w�itten notice. In the event of such terminatian, there shall be no interru�tion in
services for C�UNTY sponsored Campers already enroiled in day camp thmugh the pe�iod paid f�r
by the Day Camp Sc�c�la3ship Voucher iss�ed and paid. L3AY CAMP PROVIDER has thirty(34�
days after the effective date af the terminatian for cc�nvenience to bill far any paymenis c�ue. DAY

CAMP PROVIDER shall be entitied to receive just and equitable compensation for any services
per�'ormed hereunder thmugh the date of termination.
(b) Either party may terminate this Agreeinent in writing when it has been detennined
that the o�er party has failed to comply with any of the teims specified herein or has violated any
stipulations of this Agreement. The abligation of COUNTY to make all paymen�s due under this
Agrcement for all enrollments prior to the effecrive date af tennination of this Agre.�ment shall
survive tern�ination unIess DAY CAM�'PROVIDER fails ta perform. If DAY CANff' PROVIllER
fails to perForm,in whole or in part, COUNTY may serve a tertnination notice that shall be effective
as provided in Section 6 herein. In the event of such termination, COUNTY shall be lial�le for
payment anly for s�rvices rendered prior to the effective date of termination. Finat bzlling for
payment must be received by CQUNTY wit�in ttiirty(30)da.ys of termination date.Refunds due far
any paymen#s made for services that have not been pmvided must be received by COiJNTY within
thirty (30} days of the terrnination date. The ohiigation of DA"S�' CAMP FROVIDER to refund to
C4UNTY any payments made for seivic:es that have not been provided shall survive termination of
thi5 A�reement. ,
Sect�on S. Indemuification.
{a) Each party to this Agreement is responsible for all �ersonal injury and property
damitge attributdble to the negtigent acts of omissions arising out of this Agreement of that party
and the afficers, emplayees and agents thereof.
(b) The parties further agree that nothing contained herein shal� be construed or
interpreted as denying to any party aciy remedy or defense available to such parties under the
laws of the State of Florida, nor as a waiver of sovereign irnrnunity of CQIINTY and DAY
CAMP PROVIDER beyond the waiver provided for in Section 768.28, Florida Statutes. '

(c) The waiver of a provision herein �y either �arty shall nat cc�nstitute th.e further
waiver of s�.id provision ar the waiver of any ather pr4vi�ions.
Section 9. Assignmeut�d Third Party Be�eficiaries. Ass�,gn��nt t�f this Agreemer�t by
either party is e�cpress�y pmhibited. There arc nc�third pariy beneficiaries tc�this Agreement,
Section 1Q. M�da#ed Condftions.
{a) Governing Law. This Agreement shall ba governed by�zd co�strued in accordance
with the laws of the State of Florida and the p�a�ties consent to venue in the Circuifi Cgurt in and for
Seminale County, Floricla, as ta St,ate ac:tic�n� and the United Stat.�s District Court for the Middle
Dist�rict af Florida as tc�Federal acbons.
(b} Accuracy af Submittals. The validity of this Agreement is s�bject to the truth and
ac�curacy nf all the information,representatians, and ma#erials submitted or pravid+ed by the DAY
CAMP PROVIDER in this Agreement, in any Iater submission or response tr� a COLTNTY
request, or in any submissian or response ta fulfill the requir�ernents of this Agareement. A.11 of
said infarmatian, representat�ons, and materials are incorpcyrated by refer�nce. The inaccuracy of
th� submzssions or any materiai changes shail, at the option of COUNT�i'` and with thirty {3dj
days vvritten natice ta T?A'Y CAMP PROVIDER, cause the termination of this Agreement and
the release of Ci�UNTY from a11 its o�bligatians to I�AY CAMI� PRCMVIDER.
(c) Connte��arts. This Agreement may be executed in any nwnber of counterparts,
any one af which may be taken as an original,
(d) Clther ,Regulations. DAY CAMP' PRCIVIDER agrees to cc�mply with the
Americans Vijith Disabilities Act (Public Law lq1-336, 42 U.S.C. Section �2101 et sec�.) and the
Florida �ivil Rights and Fair Housing Acts {Sections 7b0.01-760.37, Floz�ida Statutes}, which
prohihit discrimination by public anr� private entities on the basis of disability in emplayment,

publ'rc accornmodations, transportatian, S�ate and local government services, and
telecammunications.
(e) Unauthorizec� Workers. COUNTY will not intentionally award publicly-funded
contracts to any contractor who knowingly employs unauthorized alien work+e�, constituting a
violation of the einployment pravisians wntained in 8 U.S.C. Secrion 1324a(e) [Sec#ion 274A(e)
of the Immigration and Nationality Act {"iNA")]. C4LTNTY shall consider the employr�ent by
any contractor of unauthorized aliens a violat�an of Section 274A(e} af the INA. Such violat�on
by DAY CAMP PROVIDEit.of the employment provisions contained in 5ection 274A(e) of the
INA shall be grounds for unilateral cancellation of this Agreement by COUNTY.
(� Lobbvine Prah�.`bition.
(1) No funds or other resources recei�ed from COUNTY under this
Agreement may be used directly or indirectly to influence legislation or any other official aetion
by the Florida Legislature or any State agency. ,
(2) DAY GAMP PROVIDER certifies,by its signature to this Agreement,that '
to the best af his or her knowledge and belief•
(A) Na Federal appropriated funds have been paid or will be paid, by
or on behalf of DAY CAMP PROVIDER, to any person far influencing or attempting to
influenec an officer or employee of any agency, a Member of Congress, an officer o� employee
of Congress, or an employee of a Member of Congress in eonnection with the awarding af any
Federal contract, the making of any Federa� grant, #he making of any Federal loan, the entering '
into of any cooperative agreement, and the extension, cantinuatian, ren�wal, arnendment or
modification of any Federal contract, grant, loan or cooperative agreement. .
{B) lf any funds ather than Fec�eral appropriated funds have been paid
or wilf be paid to any person for influencing or attempting to influence an ofFicer or employee of

Sec.tian 11. Entire Agreemen�. This Agreeatent consti#utes �e entire agreement betweer�
the pariies hereto with respect to the specific�atters cantained herein and supersedes aI1 pxavisions
discussions,undezstandings azzd agreaments relating hereto and cannot b�chmiged except in writing
signed by each of the parties to this Agr+e�ent.

EXHIBIT A
SCOPE OF SERVICES
Generallv:
The Community Services Block Grant - (CSBG) includes a $362,949 appropriation for Seminole
County through the Department of Economic Opportunity (DEO). Of this funding $159,136 has
been approved for a Day Camp Scholarship Program. The purpose of the Day Camp
Scholarship Program is to provide a safe environment for youth to enable working parents to
maintain employment during the summer months. Additionally, this program will enable youth to
avoid risk-taking behavior for a defined period of time.
The funds under the program are intended to target families that meet the following criteria:
• Household must reside in Seminole County;
• Household's income must be at or below 125% of the Federal Poverty Limit (see chart
below); and
• At least one adult household member is employed.
In addition, the household's child(ren) must meet the following criteria to be determined eligible:
• Child(ren) must reside in Seminole County(at least 50% of the time); and
• Child(ren) must be between the ages of four(4)and fifteen (15).

Specific Obliaations:
1. SUBRECIPIENT shall reserve at a minimum the amount of spaces for eligible campers
as specified in the Inter-Agency Day Camp Agreement, Exhibit B — Day Camp Provider
Matrix.
2. The SUBRECIPIENT will provide services listed by their agency as outlined in the Inter-
Agency Day Camp Agreement, Exhibit B - Day Camp Provider Matrix, for the equivalent
of a Day Camp Scholarship.
3. COUNTY shall ensure that the households receiving assistance meet all of the following
conditions:
a. Households receiving assistance meet the program eligibility as described
above.
b. Process invoiced payments within 30 days of receipt.
4. Administrative Costs. There will be no Administrative Cost reimbursements connected
to this agreement.
5. No SUBRECIPIENT board member, employee or any related family member of either
shall receive or obtain gain, profit, or benefit in the form of a commissions, fees, or
payment for any other related product or service funded under this Agreement.
6. SUBRECIPIENT shall submit completed monthly reports for the months of June, July
and August of 2013 in the form of Exhibit C to the Inter-Agency Agreement to the

COUNTY by the fifth (5�') day of July, August and September for the previous month.
The following categories apply:
a. Number of jobs created or retained.
i. Tracks the jobs created or retained because of the receipt of CSBG
Day Camp Scholarships.
ii. Track the amount of hours each job (position) worked on a weekly
basis.
b. Number of campers receiving a Day Camp Scholarship for the month and
how many completed the program.
i. Measures program success in contributing to parent's maintaining
employment and youth avoiding risk-taking behavior for a defined
period of time.

Thank you for choosing Camp Sunshine. The City of Winter Springs Parks and Recreatian Department
would like to welcome you and your child to aur Carnp Sunshine sumYner camp program. Our goal is to
provide yaur child with a fun, safe and memorable summer camp experience

Parents Meeting:
We will have a parents rneeting prior to the start of Summer Camp where parents can have the opportunity ta
meet aur Summer Camp Team, ask questions, and get a better understanding af our summer camp program.
This meeting will be on Thursday May 30`�' from 6pm-7pm at The Winter Springs Senior Center, which is
located next door ta the Civie�enter. Paren#s that attend wi11 be entered into a drawing for$25 off one week af
camp. Four of these $25 coupons will be given away at the parents meeting.
Camp Hours:
Camp hours are 7:3Qam-5:30pm, Any parent picking up their child after 5:30pm will be charged a$S.00 late fee
per child for each S minutes late. This palicy will be strictly enforced.
Location:
Camp Sunshine will be located in our Civic Center, at 400 Edgemon Avenue Winter Springs, FL 32708
Camper Drop-off and Pickup:
We strongly suggest to parents that the first day of camp you walk your child to his/her group and introduce
yourself to the Camp Director so they can get to know you. Parents will be required to sign in and sign out their
child each and every day. If for any reason you will be late in dropping your child off or you will be picking
them up early, please let the Camp Director know one day in advance. If someone else will be picking up your
child,please let the Camp Director know in advance. Identification will be required for everyone picking up a
child.
Registration:
We will begin accepting registrations on April 13`h, 2013 from 8:00 a.m. 11 am. at the Civic Center as part of
our Winter Springs Resident Only Registration. Open registration will begin on Monday April 15�' at the
Central Winds Park Admin Office, from 8:OOam-S:OOpm, Monday through Friday.
Weekly Flyer:
You will be given an outline of activities on Friday for the following week. Some activities require campers to
bring items from home, such as a bathing suit, towel, etc. Please make note of these days so campers can
participate in all activities. Please arrive on time for all day trips to board the bus. All Campers must be at camp
by 9am on Wednesday's for the Weekly Field Trip.
Field Trips:
Signed permission slips must be on file. All campers are expected to go along on the field trips, as they are a
scheduled activity of the Camp Sunshine Program. The cost of the field trip is included in the camp fee. Please
be sure your child wears their Camp Sunshine t-shirt on highlighted field trips and please arrive on time. All
campers must be at camp by 9am on Wednesday's for field trips.
Helpful Hints for a Great Summer
1. Please make sure your child eats a good breakfast before camp.
2. Have you child wear comfortable clothing and athletic shoes with socks. Campers will participate in
many games and activities both inside and outside. Camp staff will notify parents on days that
swimming attire is needed.
3. Parents are strongly encouraged to apply sun block before sending your child to camp each day.
4. Lunches must be provided by the parent for each day. Morning and afternoon snacks should be
planned for by the parent. Do not send food items that need to be refrigerated or micro-waved.
Please put your child's name on any personal items.
5. Parents must sign their child in and out of camp each day. If you arrive late or plan on picking up your
child early from camp please notify the Camp Director.
6. Some field trips allow campers to bring additional money. If you wish to provide your child with
additional spending money please send it with them on the day of the trip. If sending extra spending
money please put it in an envelope with their name on it. (The Parks and Recreation Department will not
be responsible for any lost money)
7. Campers should not bring items to camp that can easily be misplaced, lost or damaged, such as small
electronics, toys, and games.
