The STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION & CITY OF WINTER SPRINGS desires to supplement the
original Agreement entered into and executed on August 14 , 2009 as identified above. All provisions in the original
Agreement and supplements, if any, remain in effect except as expressly modified by this supplement.
The changes to the Agreement and supplements, if any, are described as follows:
PROJECT DESCRIPTION

Termini SR 434 and Hayes Road
Description of Work:
Construction of a right turn, deceleration lane on eastbound SR434 at Hayes Road. The taper for the turn lane begins approximately
350 feet west of the Hayes Road center lane. The project includes the relocation of the existing sidewalk, drainage structures, and
sidewalk ramps with detectable warnings, signage, and pavement markings. The project length is 0.05 miles.
SPECIAL CONSIDERATIONS BY AGENCY:
Pursuant to Paragraph 4.0 of the Local Agency Program Agreement, the Agency and the Department acknowledge and agree that the
schedule of funding for construction that includes the authorized and encumbered federal funding for this American Recovery and
reinvestment Act (ARRA) project shall be increased to an amount equal to the Agency's construction contract award amount of
$141,998.00 plus the amount of the Agency's original CEI estimated amount of $21,035.72, for a total amount of $163,033.72. Said
revision is reflected on the Schedule of Funding, attached hereto and incorporated herein, as Exhibit "A

SUBJ: FDOT Local Agency Program Supplemental Agreement — SR 434 and
Hayes Road
Please request that the City Manager sign all three copies of the enclosed
agreement, along with one witness signature and the City Attorney's signature.
The signature page is on page 3. Please return all three copies to me and I will
forward them to FDOT for their signatures. FDOT will return one fully executed
copy to the City.
Please try to have these documents signed by November 24, 2009.
Authorization to enter into the LAP Agreement was approved by the City
Commission at the May 26, 2009 meeting, consent item #201. This amendment
increases the dollar amount of the agreement based on the actual costs of the
work.
