REQUEST: The Parks and Recreation Department is requesting City Commission approval of the
Application and criteria for a scholarship (financial assistance) program for the
Summer Program and fall, winter, spring and summer classes and programs and
establishment of a scholarship account with initial funding of $800.00.
PURPOSE: The purpose of this item is to have a written financial assistance program with
application and criteria and an account with initial funding of $800.00.
ISSUE ANALYSIS:
. The Parks and Recreation Department has provided scholarships when requested and funds were
available for the summer program and some classes and programs.
. A written application and criteria is needed along with a designated scholarship account and
initial funding.
. The summer recreation program generated a profit of$I,600.00. $800.00 of that profit is being
recommended for initial funding.
. The Senior Corporation has given funds for scholarships in the past and other non-profit
organizations may also provide funding when requested.

. The Healthy Meals for Americans Act of 1994 P.L. 103-448 amended section 9 (B) (2) (C) of
the National School Lunch Act 42 D.S.C. 1758 (B) (2) (C) does not authorize the disclosure of
children's free and reduced price eligibility status for local health and educational programs or
other local level activities.
. Staff is recommending current verification be shown to staff by the requestor or verbally
conveyed from any agency that provides assistance to low and moderate income families.
(Examples: Food Stamps, Housing, Medicare, etc.)
FUNDING:
$800.00 is necessary, which is 1/2 of the profit generated from the 2000 summer recreation
program.
RECOMMENDATION:
Staff is recommending that the City Commission approves the application and criteria for
a financial assistance program and approves a scholarship account with initial funding of
$800.00.
IMPLEMENTATION SCHEDULE:
Begin using November 1, 2000.

FINANCIAL ASSISTANCE APPLICATION
The City of Winter Springs Parks and Recreation Department is a major provider for
recreational and social services to youth, adults and families in our community. In order
to provide all members of the community with an equal opportunity to take advantage of
our services, the City of Winter Springs Parks and Recreation Department has a limited
amount of financial assistance available. Individuals and families who are unable to pay
for the full cost of a City of Winter Springs Parks and Recreation program fee may apply
for a reduced rate and may be awarded partial financial assistance based on their
demonstrated need.
ELIGIBILTY
1) Financial assistance will be granted on the basis of financial need. The family
income guidelines used by the Parks and Recreation Department will be a verification
of any assistance you currently receive that is for low and moderate income families.
2) Financial assistance will be reviewed for eligibility, for each program period (requests
must be made for each individual program)
3) The entire Financial Assistance Application MUST BE FILLED OUT
COMPLETEL Y to be considered.
4) Financial assistance is not guaranteed on an ongoing basis and MUST be reapplied
for before each additional/new program. All current and correct paperwork must be
submitted every year.
HOW TO APPLY
1) Applicants must complete all Financial Assistance Application forms as well as the
standard Parks and Recreation program forms.
Your application will not beforwardedfor approval without above paperwork
2) If possible, the Program Supervisor will meet with you when you present the required
documentation. If not, you may be called to discuss the request.
3) Application processing time is typically 5 to 10 working days after all required
information has been received.
4) Financial assistance eligibility will be based on verification of any assistance you
currently receive that is for low and moderate income families and an interview with
the applicant. The Parks and Recreation Department reserves the right to deny any or
all assistance to any applicant.

Federal education programs; St:J.te tested nutrition programs with eligibility standards comparable to
school mt:als programs; the Comptroller General of the United States; and law enforcement officials
investigating violations of the programs under me National School Lunch Act of 1966 or programs
authorized access to free and niduced price eligibility information. No other infonmtion from the free
and reduced price meal application or the direct certification procedures, such as the household's
participa~ion in the Food Stamp or the Temporary Assistance for Needy Families Program, may be
divulged, without parental/guardian consent.
To facilitate low-income households" access to other programs and benefits, the 'lJSDA has authorized
State agencies to an ow school districts to disclose children's free and reduced eligibility status for uses
specified above.

The statute does not authorize the disclosure of children's free and reduced price eligibility status for
local health and education programs or other local level activities; nor does it include Federal health
programs; such as Medicaid or the Children's Health Insurance Program (CHIP). Ultimately, the use or
disclosure of children's free and reduced price school meal eligibility' is determined by the school food
authority pursuant to statutory provisions.
Enclosed for your information is the USDA prototype agreement which may be used to document the
disclosure of chil~ren's free and reduced price school meal eligibility status to appropriate officials.

DISCLOSURE OF FREE AND REDUCED PRICE INFORMATION

I. PURPOSE AND SCOPE
,Insert name cf determining agency) and (insert name of receiving agency) acknowledge and agree
that children's free and reduced price meal and free milk eligibility information obtained under
provisions of the National School Lunch Act (42 USC 1751 et. seq.) or Child Nutrition Act of 1966 (42
USC 1771 et. seq.) and the regulations implementing those Acts is confidential information. This
~ Memorandum of Understanding (MOU) is intended to ensure that any information disclosed by the

free and reduced price meals or free milk will be used only for purposes specified in this MOU and that

penalties for unauthorized disclosures of this eligibility information.
II. AUTHORITY
Section 9(b)(2)(C)(iii) of the National School Lunch Act (42 use 1758(b)(2)(C)(iii)) authorizes the
limited disclosure of children's free and reduced price meal or free milk eligibility information to specific
programs or individuals. without prior parent/guardian consent. Additionally, the statute specifies that
for any disclosures not authorized by the statute, the consent of children's parents/guardians must be
_~. obtained prior to the disclosure. For any unauthorized disclosure of children's eligibility information, the
, 5tatute specifies a fine of not more than $1,000 and imprisonment of not more than 1 year. or both.
III. RESPONSIBILITIES

IV. PENALTIES
Any person who publishes, divulges. discloses, or makes known in any manner, or to any extent not
authorized by Federal law (Section 9(b)(2)(C) of the National School Lunch Act; 42 USC 1758(b)(2)(C))
'or a regulation. any information about a child's eligibility for free and reduced price meals or free milk
shall be fined not more than a $1,000 or imprisonment of not more than 1 year or both.
V. SIGNATURES
The parties acknowledge that children's free and reduced price meal and free milk eligibility information
may be used only for the specific purposes stated above; that unauthorized use of free and reduced
price meal and free milk information or further disclosure to other persons is prohibited and a violation
of Federal law which may result in civil and criminal penalties.
