REQUEST: General Services Department (Land Management Division) presents for
informational purposes, the site plan for Duncan Realty.
PURPOSE: The purpose of this Board item is to inform the Commission of developing along
State Road 434, namely the southwest corner at the intersection of Hayes
Road and State Road 434.
CONSIDERATIONS:
This item is being presented to the Commission for informational purposes. The
Commission has previously requested that all development (site plans) along
State Road 434 be presented to them for informational purposes.
This plan is to remodel the existing structure on the property and convert this into
a funeral home. Both the zoning category and the future land use for this property
is commercial. A funeral home is authorized under Section 20-232(64) ofthe
Code of Ordinances.

The Site Plan Review Board, at the meeting of February 07, 1996, voted to
approve the project as submitted. The following caveats are attached to the
approval:
1) The entryway on State Road 434 is to be right turn in/right turn out only, and
must be approved by Florida Department of Transportation.
2) The concerns addressed in the Utility Director memo of January 29, 1996
and the City Engineer's letter of January 31, 1996 must be adhered to.

The above referenced was held on February 7, 1996, M. Tubbs and 1. Herbert represented the
project. Staff members present were Grimms, Houck, Jenkins, Ketteringham, Lallathin, LeBlanc
and Lockcuff

home.

approved by Florida Department of Transportation.

NOTE: The City Commission has stated that they wanted to see all those projects located along

Dear John:
The engineering site plan, sealed January 24 (in error), 1996,
was received on January 23, 1996. An engineering review finds this
site plan to be in general conformance with the Land Development
Code.
Upon completion of the project, you will need to furnish this
department with one (1) set of signed, sealed and dated "as-built"
plans,' a Letter-of-Certification and $300.00 (three hundred
dollars) total for the "as-built" review-inspection fee.

Duncan Realty Site Improvements - Hayes Road and Moss Road
We have reviewed the site plan signed and sealed January 24, 1996 and have found all previous
comments satisfactorily addressed. A completed Industrial Waste Permit application will have to
be provided with the meter application. We recommend approval of the proposed site
improvements.
The proposed funeral home use will require 2 Equivalent Residential Connections of water and
sewer capacity at $2,282 each for a total capacity charge of $4,564. There will be a charge for
the Hayes Road service line jack and bore that will be based on actual costs oftime and materials.
lbis is generally around $150. Other fees are the $90 meter fee, $200 customer deposit
(refundable after 24 months of good payment history), $20 sewer inspection fee, and $10
application fee.

I have received the memo and inspected both Duncan Realty and
Tuscany Place. The Duncan Realty property as shown I would make
the following comments;
a) Intersection of driveway and SR 434 right turn only.
b) If change of driveway or any new cuts made to SR 434
would have to have Florida Department of Transportation
approval.
Tuscany Place project the only comment that I have would be to
ensure that Handicap parking places are in place, and correct
amount for each building.

The Fire Department has reviewed the above referenced site plan as submitted on
December 1, 1995. The Department has no objections to the plans, however,

REQUEST: General Services Department (Land Management Division) presents for
informational purposes, the site plan for Duncan Realty.
CONSIDERA TIONS:
On February 21, 1996, the office of Land Management received the Florida Department of
Transportation (FDOT) Proposed State Highway Access Connection for the Duncan
Realty project. This connection differs from that which was recommended by the Site
Plan Review Board in that the proposed entry from S.R. 434 presented during Site Plan
Review will not be authorized by FDOT. Not shown on the plan furnished with the
Commission package are two (2) driveway cuts, off S.R. 434 between that property being
considered for the proposed funeral home and Cumberland Farms. FDOT has
recommended that the driveway cut closest to Cumberland Farms and that cut closest to
Hayes Road be closed. The only S.R. 434 entry to the property which FDOT will approve
is the center driveway cut (please see attached). This cut will allow traffic to
ingress/egress the property from both directions. The Police Department has reviewed
this approval and offer no objection.
This proposed FDOT approval will necessitate a field modification which will realign the
approved ingress/egress with the proposed project. This will only require the City
Engineer's approval before construction can commence. The Engineer's approval, besides
meeting Code requirements, will also require that Duncan Realty grant an easement in
perpetuity over that property not being developed to the proposed funeral home portion
for the placement of the roadway leading to it.
There are no other changes at this time.
