FSR.B6 I , , . .of (j THE ORANGE COUNTY/SEMINOLE COUNTY/ALTAMONTE SPRINGS/MAITLAND FIRST RESPONSE PROGRAM EXECUTIVE SUMMARY - /n l_) FROM THE FIRST RESPONSE COMMITTEE January 17, 1992 ! f. , . . , r- i ) INTRODUCTION For a number of years, fire departments in both Orange and Seminole County have realized the benefits of having agreements in place that allow for the closest unit or units to respond to the scene of a fire or rescue emergency. Now comes the opportunity to expand this "joint" or "firstll response program across county lines. In early summer of 1991, talks began between the Orange County and Seminole County Fire/Rescue Divisions to make such a concept a reality. Realizj.ng the potential for a first response program . .. along the entire Orange/Seminole County line, the cities of ( ) Maitland and ~ltamonte Springs were ~n:,ited to join in the planning -~ process to brlng the program to frultlon. In late October, a committee was established by the Fire Chiefs of the affected departments to work out the details of getting a first response program implemented by February of 1992, This work remains on track with February 3, 1992 targeted to be the effective "start-up" date. The following is a summary of the guidelines worked out to establish the program. I i . .r-.......~ METHODOLOGY , I When the First Response Committee began meeting to work out the details of the program we made the decision to use the term "first response" rather than "joint response" since we will be working on the premise that the closest unit(s) will respond to an alarm. The goal of the Committee was to develop the necessary procedures and handle the numerous details of getting the program on-line and making it work. The Committee was truly a working committee with task areas assigned to each individual for development. These areas were: - RESPONSE AREAS MOVE-UPS COMMUNICATIONS OPERATIONS MEDICAL TRAINING REPORTING - " ,-- ) In order to meet the short time frame to get the program , { implemented, we have not attempted to address every possible . ~~ j situation that might arise but rather have strived to overcome any obstacles with the intent of "ironing out" any glitches once the program is in place. In other words, follow-up will be needed to make sure that the program is working as designed. It ./" ., would take a great deal of time to conduct detailed time trials so ,I ) the committee elected to use some time trial data and then make a \.-" calculated estimate of the response area borders. The overall goal was to have the closest unites) respond to an alarm. The following is a list of guidelines used to develop the responses and response areas: *The closest unit (s) will be dispatched to an alarm regardless of jurisdiction. *All agencies will participate in a move-up plan to maximize coverage and the use of resources. (See MOVE-UP section of this report for more details.) *For the purposes of the first response program, squads and rescues are interchangeable. *If two different agencies response time is the same to a given area, the agency having jurisdiction will be the unit that is sent first. ! -3- *If response times for 3rd and 4th due units to a given C') area are close, an attempt will be made not to deplete a city or neighboring agency, but rather to use an agency that does not yet have a unit committed. For example: 1st due E-22, 2nd due E-11, 3rd due T-12 and Maitland equa 1, Maitland would be sent before T-12 and if Orange County had a unit closer than T-12, they would be dispatched before T-12. *In the event that the zone boundaries are out of line or need adjusting, the following procedure will be used: The problem area will be identified in writing with a recommended solution included and sent through the chain of command to the depart- ment's appropriate First Response Program "response area". representative. They in turn will take it to the First Response Committee for action. The "response area" representitives for the departments are: Kent Castleman, Battalion Chief Seminole County FirelRescue Dave Halstead, Battalion Commander (~) Altamonte Springs Fire Department Dan Hardester, Assistant Chief Maitland Fire Department David Hepker, Assistant Chief Orange County Fire/Rescue The developed first response areas have been put onto a large map which covers the affected areas along the county line. These have been reduced to 8 1/2 X 11 pages for the purpose of this summary and are located on the last 11 pages of this report. The dark lines indicate the borders of particular areas, circles with numbers in them indicate fire stations and the responses read like this: 64-1 23-2 45-3 63-4 The first number is the station number and the second number is in what order they are due to that particular area. In this case Station 64 would be firs~ due, station 23 second due etc. I -4- Map books from each communication center are being provided to the {-"\ units involved. The committee identified the need for companies to ~) review and be familiar with the n~w areas they will be responding to in order to positively impact response times. MOVE-UPS Along with the developed responses, another important part of the first response program is utilizing the practice of moving units up to centralized locations to maximize coverage. The following guidelines have been developed for move-ups in our first response program: *The agency having the need for a resource will request specifically what type of unit is needed and advise at what location. *The communications center of the agency move-up is being requested of will check its status screen to determine if enough units are available to send a "move-up" response. If at all possible the move-up request should be accommodated. I' ') *The agency requesting move-up shall take into z,__ consideration the length of time normal units will be unavailable before requesting move-up (i.e., move-ups of short duration may not be warranted). *The goal will be to provide as much "overall" coverage as possible. *Move-ups should be made, whenever possible, to station facilities rather than to intersections or other "open" geographic locations. *"Out of County" or "out of City" move-ups should receive priority when 'demobilizing so that these units are released as early as possible. *The communications centers will need to advise the units responding to move-up the appropriate TAC to respond on and give them the location they are being moved-up to. -5- I 1 () COMMUNICATIONS To make the first response system work, there is no more critical aspect than communications. The "closest unit dispatched" philosophy will only be successful if there is a timely exchange of alarm information with no delay. A number of hardware issues still need to be resolved so that 1) the status of units information can be shared and readily accessible to the communications centers 2) we have coverage over all of the areas where a first response may occur 3) communications centers are linked so that there is no delay in talking with one another to pass along information 4) Seminole County has the ability to perform the dispatch function from its consoles on ORANGE COUNTY TAC 2. Listed below are the guidelines that will be used to conduct communications in the first response program: *The communications center who has ultimate jurisdiction of an alarm will handle the radio communications after dispatch except: 1) when system malfunction or coverage problems prevents normal channel usage; 2) in- the ,east portion of Seminole County "") where ORANGE COUNTY TAC 2 may not be t.... effective and it is necessary to use ORANGE COUNTY TAC 3. *Calls inside Seminole County which require units from Orange County or Maitland will be operated on ORANGE COUNTY TAC 2 by the seminole County Communications Center. *Calls inside Orange County which require units from seminole County will be operated on ORANGE COUNTY TAC 2 or 5 by the Orange County Communications Center. *Standard "open" dispatch terminology will be used to avoid any confusion. Examples of this terminology include: "on the air", "available", "on the scene" etc. *when units respond on a "first response program" alarm they will include in their "responding" transmission the jurisdiction they are affiliated with. For example, if Engine 22 is responding to an alarm in Orange County, they would say, "Seminole County Engine 22 responding". Once they have done so on their initial responding -6- I message it will not be necessary to use the jurisdiction (') again. For example, their arrival transmission might be, "Engine 22 on the scene, nothing showing." *When a request for a response is made, the initiating communications center needs to advise the other communications center of the type of alarm, the location, the map page, the TAC the unit is to respond on and whether there is a deviation from responding "emergency". *The level of response (type and number of units) and the type of response (emergency or non-emergency) will be determined by the communications center having jurisdiction of the call. *The communication centers identification for radio transmission purposes will be "SEMINOLE" and "ORANGE COUNTY". As with other aspects of this project, there will have to be adjustments made once we go on-line with the system. Before going on-line, however, we need to make sure that each center has the ability to communicate effectively with field units. - () OPERATIONS addressing In a first response program, one of the main considerations are the operational procedures to be used. In the operations area we needed to look at how the departments differed so that problem areas could be resolved. We found no significant differences and came to the conclusion that with some education on the front end as well as some adjustments in the future when we were on-line with the program, operations would work fine. Listed below are some general guidelines going into the program: *All alarms dispatched will be responded to emergency, unless identified as non-emergency by the communications center. Based upon information given at the time of dispatch, the company officer has the authority to request additional equipment and/or upgrade the response to emergency status. *Brief arrival and situation reports are expected and should include the situation, type of structure, percentage of involvement and type of attack lines being used. Establishment of a water supply should be an -7- ! immediate consideration and if possible included in the c-"'} primary report for the second due company to establish. *On vehicle accidents, the primary goal of the-report should be the number of patients, whether they are BLS or ALS patients and if additional resources are required. *On general EMS type alarms, the reports should be brief advising patient stability, whether they are BLS or ALS and to which hospital and by whom the patient is being transported. *The. officer on the first arriving unit shall assume command of an incident until relieved by a higher ranking officer. Command does not need to be "established" on general EMS alarms, auto accidents, small fires etc. where an engine or a rescue are the only response units. The general rule should be that when three or more fire department units are working together, command should be established. *Full protective clothing shall be worn by all personnel when performing operations other than general EMS alarms, unless otherwise authorized by the incident commander. *The second- arriving engine shall be responsible for () locating the water source and shall be prepared to " immediately establish a water supply. ~ _./ *Companies with the capability should use valves that allow them to pump the hydrant. On commercial building alarms the engine should automatically return and pump the hydrant. *Companies responding out of their normal jurisdiction to treat patients will treat all patients according to their own EMS protocols/procedures/standing orders. *Company officers assuming establishing command at an alarm are considered "working" officers and should perform their designated functions as normal giving reports and directions as possible. For example: on a typical residential fire the company officer assumes command upon arrival and makes the decision to "establish" command. The officer would then proceed with fire attack responsibilities and provide updated reports and direction as possible. A command position in front of the incident should not be established unless there is \ -8- sufficient manpower to allow for this, or the type of C; incident dictates this type of action. *staging and awaiting law enforcement on assaults, shootings, stabbings, etc. , will not 'be done on an automatic basis. The company officer shall obtain all information available from the dispatcher, approach and observe the scenelconditions and make the decision whether to proceed in or stage and wait for law enforcement. The exception would be if law enforcement is already on the scene working the incident and requests the units to stage. *Problems that arise in the area operations shall be resol ved at the lowest level of supervision possible. If unresol ved they will be forwarded up the chain of command for resolution. In addressing such issues, the guide should be that: problems shall be resolved in a "spirit of cooperation." Additional operational issues that arise shall be addressed by the First Response Committee during periodic meetings. MEDICAL - . /) or The four jurisdictions in the first response program operate under three different sets of protocols/procedures and under three different medical directors. This is not proving to be a problem in this planning phase as we have been able to handle alarms under this format on mutual aid calls in the past. There has also been a spirit of cooperation on this issue on the part of the EMS managers, as well as the medical directors, so in the area of medical response there appears to be no obstacles. The following are some general guidelines that are being established: *The long term goal is to consolidate the three sets of medical protocols/procedures/standing orders into one document. *On calls involving ALS, the first paramedic to begin treatment on a patient is considered in charge of that case. *prior to ' consolidation of standing orders, two operational guidelines will be observed: , -9- i 1) whenever a fire department paramedic (-) institute7 therapy, that deviates from ~he , . transportlng agencles protocol, the flre department paramedic must accompany the patient to the hospital and. continue responsibility throughout the pre-hospital phase; 2) whenever a particular agency's protocol furthers the treatment in a particular case, that treatment will be allowed. *An EMS subcommittee, with the express purpose of maintaining quality assurance, will mee.t regularly to review calls and issues. *County jurisdiction will prevail in matters concerning the private providers of ambulance transportation. Ambulances will not automatically be sent to another County. If a need is perceived by fire department personnel, another county's ambulance provider may be requested on a "special call" basis. TRAINING - () One key factor in the success of any venture, is having personnel knowledgeable in how a program will work. To educate personnel in the first response procedures a number of steps have been or need to be taken. These include: *Large scale evolutions at the Maitland Sheraton were organized for all three shifts involving some of the stations that will be responding together in 'the first response program. These drills were held on January 3, . 10, 20, 1992 and provided an opportunity for personnel to become familiar with procedures and equipment. *Orientation training which will be held with each department by their representative members to cover the basic first response procedures such as communications, EMS procedures, response areas, etc. *Map book training which will be cond~cted within each department's normal . training procedures to facilitate personnel learning their new response areas. . -lO- r *utilizing the "joint response" training group made up of (-" the area training officers as a resource for future \ J training sessions. This training would include basic drills, evolutions, equipment familiarization, area survey, extrication procedures, hazmat procedures, helicopter requests, standard operational procedures for various types of fires, mass casualty procedures, etc. *Large scale drills to include scenarios at large buildings, hazmat sites, potential transportation sites, etc. *Intercompany training exercises initiated on the company level to familiarize adjoining stations with equipment, personnel, procedures and response areas. REPORTS In looking at the issue of reports we found a mix of procedures which includes the fact that Maitland, Altamonte Springs and Seminole County are part of the State reporting system whereas Orange County is not. By looking at the different scenarios of unit response we ~eveloped the following guidelines to handle the reporting needs for first response alarms: :{ -) h "b tt I' , . t ~y/ *T e 0 om 1ne" on report1ng procedures w111 be tha if an agency responds as the only unit to an alarm in another agency's jurisdiction, they will complete reports in their normally used format and provide the agency having 'jurisdiction with a copy. - *The centralized point for reports and other inter- department correspondence will be the Maitland Fire Department located off of 17-92 on Independence Ave. Reports will need to be picked up and delivered to this location at least once per week between the hours of 8 AM and 5 PM. *An exchange of incident report information is necessary when: 1) an agency having jurisdiction is not represented with any, units on an alarm within its own jurisdiction. In this case both the NFIRS and EMS report copies would be necessary. Example: Agency "A" responds to an alarm within the -11- jurisdiction of agency "B" without the ,C) presence of any agency "B" units, or 2) an ALS rescue unit responds into another agency's jurisdiction on an. EMS alarm along wi.th the primary jurisdictional engine company. Example: an EMS call in agency "A's" area, Engine "A" responds with an ALS rescue unit from agency "B". The agency "B" unit would need to send a report copy to agency "A" for data storage. *A carbon copy or computer printed copy of the NFIRS report must be exchanged on all of the above described incidents. A NCR copy or computer printed copy of the EMS report must accompany the NFIRS on EMS related incidents. These reports must be clearly written and complete, as statistical data will be derived from the reports. *On all alarms it will be the responsibility of the communications center having jurisdiction to call the other communications center to obtain the agencies IR number. On alarms, an IR number (different) will be generated in each communications center. This (')' information needs to be included in the appropriate \"'- location on the incident reports, as well as on the first line of the remarks section. Example: On an alarm in which an Orange County unit responds as the only unit to an alarm in Seminole County, the Seminole County IR should appear on the first line of the remarks section of the report for cross-referencing. After completion, a copy of the report will be forwarded to the Maitland Fire Department "mail site". The agency having jurisdiction will be responsible for completion of the mutual aid received report. *On alarms where there is a multiple unit response the first arriving unit will be responsible for completing the incident report unless the officers on the scene agree that a different unit will assume that responsibility. *If for some reason a unit does not receive alarm times and IR numbers, they should contact their own communications center for the information. , -12- I *Questions or pro~lems with first response program (--') reports should be directed to each agency's "reporting" 1 representative. The representatives in each department are: Dan Hardester, Assistant Chief, Maitland Fire Department Tom Schaefer, Battalion Chief Seminole County Fire/Rescue Division Dennis Snyder, Communications Coordinator, Orange County Fire/Rescue Division Terry Winn, Lieutenant, Altamonte Springs Fire Department RECOMMENDATIONS The first response committee makes the following recommendations concerning the first response program: 1) The program should be implemented on February, 3, 1992, at 0800. - C) 2) The first response committee, and selected sub-committees should continue to meet after implementation to evaluate the program, handle any problems that arise and make adjustments as needed. 3) The communications centers should obtain the necessary hardware ( status screens, phone lines, transmitters, repeaters, etc.) to allow for a smooth and rapid exchange of information. 4) There should be further consolidation of operational procedures, such as: a) adopting the National Interagency Incident Management System as the system of incident command; -13- j b) adopting a unified set of medical protocols/ (~ procedures/standing orders; c) refining standardized concepts of fireground operations; d) adopting a standardized computerized reporting system should take place once the first response program is up and running. S) The training subcommittee of the first response committee needs to continually address the training needs of the first response program. SUMMARY The advent of this type of first response agreement, where for alarm response purposes the County lines are "erased", has been long awaited. During the time the Committee has been together two feelings have become manifest: 1) there has been a real enthusiasm on the pacct of the Committee members as well as with the field personnel during_drills held at the Maitland Sheraton about the first response program, and f ) \, ,,-, 2) as we worked through the details of getting the program on line we realized that there are a number of areas that can be positively impacted by the agreement that will further enhance service levels. The members of the first response committee have enjoyed being involved in this process.
