FLORIDA DEPARTMENT OF
COOPERATIVE AGREEMENT
The Seminole County Health Department and the Winter Springs Fire Department I
hereby agree to a collaborative effort aimed at improving childhood immunization levels in! the
community by making immunizations available at the local fire department.
This agreement is entered into pursuant to the authority in 381and 401 Florida Statutes and
64E-2 F AC.
SEMINOLE COUNTY HEALTH DEPARTMENT AGREES:
The Seminole County' Health Department will provide the necessary training to assure
competence in the delivery of childhood immunizations through the following:
-A written immunization protocol to be kept on -site at the fire Station for use as a resource
manual.
-A comprehensive introductory training to cover vaccine schedules, administration of vaccines,
documentation of vaccines, required forms for school and daycare centers, valid and .invalid
contraindications to immunization, screening questionnaire, vaccine information sheets and
immunization records.
-Availability of the Epidemiology for Vaccine Preventable Diseases, CDC course via satellite or
video tape followed by a written post-test.
-Hands on supervised training in the administration of childhood immunizations for a period of
several weeks or until both agencies are satisfied that the required level of competence is
achieved.
-To make available the required vaccines with instructions on proper handling and storage.

Winter Springs Fire Department agree's:
- To a schedule between the Winter Springs Fire Department and the Seminole County Health
Department allow ongoing hands on training for ParamedicslEMT in administering childhood
immunizations under the supervision of a public health nurse until adequate proficiency is
achieved.
-ParamedicslEMTs are employees of the Fire Department and shall function under the auspices
and supervision of the Fire Department Medical Director, and shall be covered by the City of
Winter Springs liability coverage as employees of the Fire Department.
- The parties to this agreement acknowledge that neither party is an agent or employee of the
other party,
- The medical record (Immunization card DH Form 687) of all patients shall be treated as
confidential, so as to comply with all federal/state statutes and Department of Health rules and
regulations. To assure this fact, each participating Paramedic/ EMT shall sign a qOH
Memorandum of Understanding Regarding Confidentiality of Client Information.

-Paramedics shall complete required training and pass an oral and or written comprehensive
post-test before participating in the Health Department Immunization Program and shall comply
with protocols and procedures relating to immunization services. .
All activities under this Agreement shall be conducted in accordance with Title VI of the Civil
Rights Act of 1964. No person shall, for reasons of race, color, sex, handicap, religious creed,
national origin or political belief, be subject to discrimination.
A joint planning and information session shall be scheduled at least yearly. This Agreement
shall automatically renew for one (1) year unless either party notifies the other party of its
intention not to renew this Agreement at least thirty (30) days prior to termination.
